How Bulk Orders Work
Download the spreadsheet, enter your roster and sizes, then email the completed file back to EZ Sports Apparel.
1. Download the Form
Click the button and save the EZ Order Form to your computer. The form is designed for bulk team, league, school, and organization orders.
2. Fill Out the Order Details
Enter the organization, team name, player name, package/item, name printed on item, jersey number, sizes, quantity, and any special notes.
3. Email It Back
Send the completed form to EZ Sports Apparel for review, invoice, production setup, and order processing.
When to Use the EZ Order Form
Best For
Large leagues, school programs, rec organizations, multi-team uniform orders, and league-wide player packages.
Use This When
One person is collecting the roster/order information and submitting everything together for multiple players or teams.
What to Include
Team name, player name, product/package name, name printed on item, jersey number, jersey/shirt size, pant or short size if applicable, quantity, and notes.
Online Team Stores
Online stores are still available and are best when families will order and pay individually. Bulk forms are best when one contact wants to submit the full order.
